Social Security & Medicare while transfer from F-1 to H1B

I'm in OPT GAP right now.

What form or document do I need to fill or proceed in order to let the employer knows that they should deduct Social Security Tax & Medicare from my paycheck starting from Oct 1st.

BTW, is a new Form W-4 necessary?
