H1B & H4 DS160 filling question

My H1B and my wife's H4 stamps have expired. My H1B and my wife's H4 are renewed and I'm in USA right now. We are going to India this April. We need to go for VISA stamping. I've a few queries regarding filling DS160.

1. Should I fill for Spouse's address in either application with India address as in Passport or USA address? Both of us have different Indian address in passport.
2. Should I provide each other's information for "any immediate relatives in USA?" question.
