doubt in end client address in ds 156 :221g

Hi I am kandhala attended for h1 and got 221g in October.
Vo gave me blue sheet and asked me client letter and other stuff.

I am going to use Drop box facility and had 3 doubts now.
I request any one to please clarify these things.

1) is i have to fill the Ds 156 with this Client address
(or)
I have to submit the old ds156 form which have my consultant address in
the field of address coloumn in ds156.

2)Is i have to fill and sign the ds156&157 new forms with latest dates?
(or) can i use the old forms?

3)How many days/weeks Consulate took for clear 221g cases.
