H4 to H1 - urgent help needed

I had applied for H4 extension, I got the approval notice & I-94 but the class & dates(valid period of stay) are missing on the documents, so the case is pending for correction now.

Can I in this case apply for H1B 2008?

What documents related to H4 are needed in this case when I apply for H1B 2008?
