Home address? -Revalidation in US

Hi,

I decided to revalidate here. What should I put in the Home address? My US Home address or my home country's?

Should I click "Yes" for the question "Has anyone filed an immigrant visa petition on your behalf?" if my employer submitted an application for green card process but haven't got any info about it, not even a labor certification?
