Very Urgent - Last Name Issue

Hi All,

I know the name issue has been a lot of discussion in this forum and I appreciate the speedy responses.

My full Name is split up as :

FirstName : Raj
MiddleName : Mohan
LastName : Kumar

All my certificates and X Matriculation certificates show the name as Raj M. Kumar. (Middle name Initial)

My passport shows the details as
Surname : Mohan Kumar
Givenname : Raj

My I797 shows up as Kumar, Raj Mohan (which I assume as lastname, firstname middlename)

When I applied for H1 Visa, and while filling up form DS156, I was confused over the lastname and surname issue and based on some advice I filled it as exactly in passport
Firstname : Raj
Middlename :
Lastname : Mohan Kumar

When I got my H1 Visa stamped on my passport, it looked exactly the same as in passport.
Surname : Mohan Kumar
Givenname : Raj

While filling out the I94, I followed the same and gave
Familyname : Mohan Kumar
Givenname : Raj

The SSN and the Driving License followed the same and issued me
Firstname : Raj
Middlename :
Lastname : Mohan Kumar

I want to get this issue resolved and have my name corrected and show up as
Firstname : Raj
Middlename : Mohan
Lastname : Kumar

Please respond urgently as I am planning to go back to India in December.
I have to file for my H1 extension and follow up the GC process once I am back, but I want this corrected before all.

What should be the procedure I need to follow to correct this issue.
