H4 Visa - Name incorrect on I-797A and I-94 forms

Hello everyone,,
My wife's H4 was approved in 2016 along with my H1 petition. It is valid till May 2019. Today we noticed that her name was incorrect on I797A/I94 form (probably it was our mistake in filling the form). Her first name was in given name and given name was in first name.
