H4-H1-H4 DS 160 Questions for Visa Stamping

Here is my case.:

-- My Wife Came on H4 visa in 2008 (valid till May 2010).

-- Applied for H1B after coming to US and got the approved petition with I-94 eligible to work from Oct. 2008.

-- She did not work any where (didnt have any pay stubs, W2 etc employer didnt pay her.)

-- She went to India in April 2009 and came back on previously stamped H4 visa in May 2009.

-- Her H4 got expired May 2010 filed for H4 extension got the approved petition (valid till Sep 2013)

-- She went to India April 2011 Now I am filling the form DS-160 for her H4 visa stamping

Now my questions are -

1. SSN number

Do I need to fill the SSN number or Does not apply because she has SSN number.

2. Are you previously employed? Yes or No

What I have to choose "Yes" or "No". If I select "Yes" below are the questions.
