Japan multiple entry visa query. Need help.

Hi folks, I am planning to apply for Japan multiple entry visa from Delhi vfs and will travel after few months if approved. In this case, what to fill in the application form in the details of 'date of arrival in japan', hotel address, etc.

I already went to japan once last month and now planning to apply for multiple entry visa to travel in future.

Please provide your valuable inputs.
