I was required to fill up the h4 application form in New York embassy. However, it was asked for me to fill up my first name, mid name and last name.
Since my name is Shirisha Devi Rajendra Rao, whereby my first name is Shirisha Devi and my father's name is Rajendra Rao, I was unable to fill up the name section.
