Adjustment of Status - I-485

Good Evening to ALL-

I hope those who passed the interview can clarify and give me a good advice.

I have a question about the medical form you submitted. I just noticed on my medical copy given to me, there is this question of "Screening Test Name" for Syphillis section and "Screening Test Name" for Gonorrhea section BUT the medical office typed instead the date of when the test was done.

Example: Syphillis
                 Name of Screening Test: 11/29/2016
                 Date Screening Run: 11/29/2016
                 Screening Non Reactive: 11/29/2016

                 Gonorrhea
                 Screening Test Name: 02/15/2017
                 Date Specimen Reported: 02/16/2017
                 Negative

I am worried now if this will be rejected because the name of the screening test was not written but instead the date when the test was done.

I hope you can share what was written on yours to give me an idea if I have to really go back to the medical office and ask them to re-do all of these. I hope I don't get charged if I ask them to re-do this again.
