please help two surnames now
my name is misspelt in my new indian passport as raj mani when the right name is raj manni .
I lost my indian passport and applied for new passport in houston texas , and they misspelt my name . iam already in mexico for h1b stamping .
in the ds 156 form , i see two fields , the first field :surnames ( as in passport ) and the second field : other surnames used ( maiden , religious , professional , aliases )

surname ( as in passport ) for that should i fill in manni ( the accurate last name )or mani ( the misspelt last name in the indian passport ) .

and in the second field : any other surnames used : should i fill in this the manni .(if i fill in the surname , mani in the passport in the first field ).

please let me know what should i do, what should i tell the consular officer about misspelt name , should i tell i changed my name or there is mistake in my indian passport , iam already mexico for h1b stamping , i didnt recognize this misspelt name before .

regards
raj
