Question for OCI application - Chicago
Hi Guys,

I am waiting on my passport to receive back with the cancelled stamp. I was looking for the information on OCI application on the VFS site along with the checklists and got couple questions and hoping if anyone can answer/guide.

1) on the VFS site, under OCI services, Step No: 3 states that "If you are part of a family or group, you’ll have to create an individual profile for each member of the family or group." Does that mean that I need to create an account for my baby as well?

2) Regarding the self attestation of the documents for minors, what's the best way to self-attest the documents for minors like a baby? Should we paste the thumb print on the documents - Left side in case of baby boy. Should that only suffice the requirement? Or Along with his thump print both parents need to sign and/or state the name? Or in place of a baby only either or both parents can put the name & sign? I am not very clear on this requirement if anyone can enlighten me on this?

3) For a name change, My first name as per Indian passport stats as "Parthkumar" but during the naturalization my First name was changed from "Parthkumar" to "Parth" only kumar is not my surname but in my passport I got "free" kumar along with my real first name as per SLC (School leaving). Should that be considered as name change? If so then, do I need to submit a notarized name of change affidavit? Or If I can include the endorsement during Naturalization should meet the requirement?
