Questions regarding Passport Submission for a work permit application
Hi,

Thanks in advance for everyone on the group trying to help out people like me.

My employer had applied for a work permit for me to move to our office in Canada. I got an email from Fragomen (my company's uses them for immigration management) that my application has been approved along with a passport request letter. I have a travel planned on 19th January and from the experiences from this group it seems like I should send the passport ASAP if I were to have any chance of getting it back in time. So I have some questions to make sure I don't make ANY mistakes.

1. On the page 4 (BIOMETRICS) of the consent form, should I fill and sign the "Applicants NOT wishing to provide biometrics" section since I am an Indian citizen and my application has already been approved before 31st of December?

2. On page 3 of the consent form, there is section for "Consent for Indirect Submission and/or Collection (to be completed ONLY if travel agent or person other than applicant themselves is submitting and/or collecting the visa application and decision)". I am sending the passport myself through mail along with a return label. Can I skip this section? The only reason I ask for a confirmation here is because my employer filled my original application.

3. Since I am just sending my passport with the consent form, passport request letter and contact information, I just need to send a money order of USD 19.90 for the Package transmission fees right? No other fees?

4. Some people have mentioned that we should send 2 copies of the consent form. Is that a requirement?

5. Is there a format for the contact information sheet or is it just a general letter with my name, email, address, DOB and phone number?
