It could have really been mishandling by USPS. We all have experiences with receiving other people’s mail delivered to your address, right? (The mail could be addressed to a different apartment or street address, etc.)

USPS often delivers mail to wrong addresses, and the recipients may return the mail by writing “wrong address” or “Does not reside here,” etc. Then, USPS will return the mail to USCIS, thinking the addressee is no longer there. (My company often receives returned mail just like that, but we know for sure that the addresses on the mail were correct and current.)

This almost happened to my friend when CIS sent her a green card, which was delivered to a wrong address. The recipient was kind enough to drive to her place and delivered the mail, which he knew would be extremely important.
