Regarding documentation for spouse application
Hello All, I need to submit passport application for my spouse.
As in most cases, my wife adopted my surname and it's also there in her current passport.
My doubt is around the affidavit for name change. The affidavit template I see in the checklist seems very generic, not specific to the women adopting husband's surname.
Is it fine to provide an affidavit explaining the marriage and the reason for name change or we have to use the given template only?
For Indian address proof, can we provide Birth Certificate which has Indian address listed. I know, this is not listed on the checklist, but my spouse does not have any document listed in the checklist.
