Adjustment of Status - I-485

Rejected I485 with some questions.
Hi guys the USCIS in Chicago received I-485 on April 16 and we called and they said they rejected our application on May 1 and was send back to us. Today, May 8, we received back our packet and it says that it was rejected for couple of reasons.
1. The payment amount is incorrect or has not been provided.
- We submitted applications I-485, I-765, I-131 in 1 packet with a check of $1,225. Was it wrong? I'm pretty sure it is correct. When we got the check, they also stamped/erased the US Department of Homeland Security (handwritten) with their own U.S. Department Of Homeland Security Citizenship and Immigration Services. Do we need to rewrite the check with the stamped name?
2. It also says that the application is not fully completed. Missing the I-944 and/or I-I864/I-864A. We are missing the I-944 because we didn't know we need to do that form. My question is then, when we resubmit our application with the I-944 and filing fee, do we need to fill out another form or just submit the same form with same signatures and dates?
