Surrender Certificate

Surrender Certificate form on VFS site
The document checklist states that "Click here to fill the form online on VFS portal and the surrender certificate will be sent across over email for print out. This needs to be duly signed and submitted" When I click the link and fill the form the formatting is off and how are we supposed to submit to VFS as there is no online submission button. How will the certificate be emailed to me? as the requirement is to attach 2 copies of this certificate.
Also the VFS order confirmation email ( which is sent to you after making the payment ) does not include any surrender certificate. I wrote my query to VFS and its been 4 days and no response.
Any guidance would be highly appreciated as its really urgent for me to apply for my OCI card.
