vaccination record
Dear all
Please on form I 693 part 7 vaccination record are we meant to do all vaccination. i have RFE saying medical doctor did not complete the vaccination portion of the form. the only record filled was TDAP ---date given MMR ---date given Influenza date given . others like hepatitis A, B , rotavirus, meningoccal etc was left blank. thou i plan to visit the clinic tommorrow since i am on my way to work but i need to get the real info clearly. thanks
