RFE for I-864 (Affidavit of Support)

I have a question, I have a RFE for I-864 about the affidavit form, I missed the part 4 in page 3 the sponsor name, I forget to put the name of my husband in that part and I have to send again another affidavit form, my question is if I have to put the alien number that USCIS already sent me with the form 1-797 notice of action, or should I leave blank as it was before?
