how to lab reports with the medical exam ?
so since I've done my medical exam on the 14th of February but doctor forgot to attach lab reports, do i need the doctor to fill new form with the dates that I had my exam at and attach lab reports to it ? because I'm going to do a motion and I don't want nothing to go wrong>>> if you wanna check my story look at my profile ..
