Same Sex Bi-National couple - Approved at interview, now being told we will receive RFE

Biometrics - Completed in Oct 2016
EAD cards mailed and received in November 2016
Notice of Interview scheduled for March 30th - Seattle WA
Completed interview with wife and daughters - Officer said" I have read through your file, and I am approving you for your I485". He gave us all the info regarding when we would receive our cards in the mail. Very happy day for our family. He told us he would update our case that day!
By 5pm on day of interview, our case was changed to "interview complete and we need to review now.
Our lawyer emailed the field office and found out that our officer made a mistake in saying we were approved and that he would be issuing a RFE. Unfortunately, officer has been out of office entire week, so we will not even receive the RFE till maybe late next week.
How could this happen after he approved us?
