1) Have you ever been issued US visa?
- if yes - Date last visa was issued ? Visa number ?
( Should this be my F1 visa or H1, if H1B then what will be the Visa number? )
2) Are you applying for same type of Visa ? ( related to question 1. )
- should this be YES or NO ?
