If one is going for Visitor Visa 2nd or later time, please take the VISA extension Approval paper (atleast the copy), if they have extended their stay during their previous visit in USA. My in-laws had this experience recently. They were asked to come back with VISA extension approval letter from their previous visit, since they were asked for the proof of their legal stay in US.
My in-laws received the VISA, when they went back with the extension approval letter.
