need help to fill DS-160
I applied H1 in 2006 quota and went to US. I applied for H1 extension and got approval for another 3 years. i went to India for personal reasons, attended interview got visa again. but unfortunately i did not travel to US bcoz of my employer. then i transferred my h1 to another company and he asked me to go for stamp again. i went to US embassy but they refused to give visa and they cancelled my valid visa. then now i applied for H1 and got approval, i need to go for stamp again.
please advise me to fill DS-160 correctly.

Yes or NO
if i say Yes it asks for
Has your U.S. Visa ever been cancelled or revoked?
what should i answer for above question? Yes or No
and another question:
Have you ever been refused a U.S. Visa, or been refused admission to the United States, or withdrawn your application for admission at the port of entry?
who has the similar kind of situations, Please help me I would appreciate your help.
