While filling the new application for scheduling the appointment there are 4 rows under petitioner
what is petitioner name ?? what exactly i need to give here(it should be employer name or his name?)
start date ? (what exactly i need to give here ?)
receipt name ( i think i need to give my receipt no)
end date ? (what exactly i need to give here?)
Can anyone please help me on this ....
