NEED HELP!!
Hi everyone,
First i will start with update: i ve been waiting for my interview to be scheduled since October 1, on February 5 my interview was scheduled to March 9. SF office.
Now i have a problem. During my I-485 application pending i got notice from USCIS that my form I-693 Report of Medical Examination and Vaccination Record has been submitted in version which is no longer in use. I ve been required to submit "a valid, completed and signed Form I-693 at a later day".
What should i do? Bring the same I - 693 form with old examination and vaccination results (july 2015) in a new version (if yes on which day it should be signed), or go through all vaccination and examination one more time and get brand new I-693 form with new dates on it? I JUST DONT KNOW WHAT TO DO. my doctor also doesn't know which one is right way, but it is her fall that she used wrong version of I 693!=( Do you think national customer service can help me with it? Should i call USCIS and ask what do they want?
