Help needed for example/sample for letter of intent for EB1A based I-485 filing.

For EB1A petitioner filing I-485 to change status, there is a need to submit a signed statement confirming you intend to work in the occupational field specified in the Form I-140.

Does anybody have such letter sample or example?

I would like to submit letter with proper language and wordings.
