The medical exam can be completed by any local Civil Surgeon. Usually a search on google will grant the desired result if you're anywhere in the 50 states. As far as the vaccines go, if you do not have any record of the vaccines, you will need to have them completed again. Even if you have had vaccines done, in some cases, like my own where my wife is from Ukraine, the standard vaccinations there are not the same as we have here example: she is vaccinated for M and M out of the MMR that we require (Mumps, Measles, Rubella), and therefore will require to have the vaccine containing all three re-administered. However, once you've had all the vaccinations and tests done, there still may be an issue. In my situation, my wife has been vaccinated for Tuberculosis. This isn't good because when they give her a TB skin test, there is a chance that the test will show positive, in which case she'll need a chest x-ray to confirm that there is no issue. You do not need to get all of the vaccinations from the same doctor, for instance, if the Civil Surgeon's facility offers one price, but you have a doctor who is willing to do it for a cheaper price, you can always have the doctor send you with papers confirming that all the tests have been done, the Civil Surgeon simply needs to perform the examination. It can be completed in the time that it takes to complete a TB test, (2-4 days).

Oh yeah, as for cost, I have a doctor who's a friend of mine so he only charged the cost of the vaccinations, which I believe comes to 170 dollars, however the local "Wellness Mart" gave me a quote of 480 dollars; so it definitely matters where you go. Good luck!
