Re-Entry Permit - Second Time

Hi
We came back from USA and settled down in india. while coming from US we have filed out Re-entry permit which is valid upto Jun-2006.
I came to know that we can get re-entry permit second time
Does any body have idea about the same
I do not want to lose my GC status
Can i go to US and file the re-entry permit again
Which form i have to file it again

What reason did you give when you filed for your first re-entry permit.
Normally they may renew it for 1year from now on
