when I would be renew my green card?

Hi!My green card is expired on 08/11/2008 after two years for permanent resident.My question is:

01.when I can possible to renew for a ten years one?
02.should I do it now or I have to wait within 3months?
03.what is form number I will use?and how much does it cost?
04.Do i have to submit any documents ?like bank account?house rental?insurance ?..etc....?

Please help ....appreciate your soonest reply...
