I am filling out I-90 to replace a permanent residence card. There is one section I am confused about. First is "Where did you apply for your immigrant visa or adjustment status?" Is this where I had my interview? Also "Where was your immigrant visa or adjustment of status issued?" Is this where my green card was shipped from? It is my first time applying for a replacement card. I am filling this out without any help, and do not want to make any mistakes that may lead to a denial. Thank you.
