I-751 verifying correct form

I have seen a couple of people whose applications were rejected because of not using the current form but the current form is dated 12/30/2019. I am filing in two weeks. Can people who have filed verify which form they use so I make sure I have to correct one? And making sure the filing address is Phoenix, Arizona? I am located in Texas by the way.
Also, any filing tip will be helpful.Thanks

Just make sure that you download the I-751 directly from the USCIS website. It seems that you are looking at the expiration date of the form. This is counterintuitive but you actually have to go by the edition date. The current edition is 12/02/2019 N. (I do not know what the N stands for).
