txh1b:Some issue regarding fuling up I-290B

Now while iam fulling up the form I-290B regarding the denial notice of my I-765(EB3),iam having some issues with some information on it.i have to make sure that every thing is right.
What is mean the folowing information on the form I-290B:
1/On the right top(part1)it says "In the matter of" should i right "I-765 "or " a Motion to reopen a decision made on I-765"
2/file number A- Does it mean receipt number of I-765 or i have to leave the blank or an other number i can see it on all my applications begins with the letter "A" which i think is a correspondence number given to me by USCIS?
NOTE: I know that the letter "A" folowing by a pound means alien number.
3/"application/form number"( part2)should i write I-290B or I-765?
