OPT EAD approved 45 days ago, still not received EAD

Hi,

I applied for OPT EAD and my case was received by USCIS on 12/10/2018.
I got the receipt notice on 12/13/2018. Then i waited for more than 4 months before my case was approved.
My case was approved on 04/16/2019, but still my online case status did not change and today also it shows that my case was received.

Now, usually the card is mailed within a week after the case is approved but i haven't received my card yet.
When i put in online case inquiry, i got a reply that my card was mailed but when i called USCIS, the representative told me that their system shows that my case was only approved and it has not been mailed. Now, they say wait for 30 more days and if you don't receive your card, then we can put in another request.

I am confused what will be the start date on my EAD card because the start date which i chose was 03/25/2019 and my case was approved on 04/16/2019. So, will the start date be when the card is produced or is it the date when my case was approved, because if it is the latter then i have already wasted 45 days of my OPT. The USCIS representative didn't have an answer for it.

Can somebody help me with this ?

Perhaps call USCIS again and ask to speak to level/tier 2 person?
They will give you crap but just keep your foot down till they do.
All Level1 can tell you is what you can see in your case status and do online.
