I-751 form help please..

me and my wife filling I-751 form to remove my condition to residence.everything is oki but after review some blogs I got confused..my spouse,(my wife) has a kid, us born, citizen and live wih us. on the form part 5,information about your children,list all your children section, should we list his name or as i thought he doesn't need any condition removel or anything and US citizen already just fill none.
second question is part 4 information about your spouse part,Family name should be my wife given name before marriage or my last name which she changed after marriage..kinda confused)
