ETA Form 9089 question

Hi Guys, i have a question regarding form 9089. I am
filling out the job experience portion of the form and i dont have the contact info for most of my supervisors. I was wondering if it would be ok to put the number for hr instead?

Yes, HR Department / Phone numbers will work. Make sure that the other entries are accurate including, address, full name of the supervisor, etc.
