Want to confirm few things on the DS-261 form. When filling out the DS-261 form, the Personal Information (Surname and Given Name) on the first page should be that of the Beneficiary, correct? There is only field for surname and give name, where should the middle name go?
Also, is the Beneficiary ID same as the Invoice ID (IIN)?
