Reasons why GC will not get approved

I am just wondering before i file my mom GC

What would be reasons fro not approving application for GC ?

She is 72 years old

She is here on visitors visa, and would overstay while she is waiting for the decision. If she gets rejection on the GC appication, will she still be able to come back on her visitor' s visa ?

If you are a US citizen, her overstay does not matter as far as gc is concerned.
No her visa would have been automatically revoked. She would have to go to the consulate in her home country for a visa, and there is a good chance that her request for visitors visa would be denied citing immigration intent, because of what transpired.
