GC Question - Marrying a US Citizen, query regarding I-130

My wife, a US citizen, would be submitting form I-130 sponsoring me but we had the following queries:

- I am currently living in US and my wife is in UK on an international assignment for around 5 mos, would her temporary resident in UK be a problem while filing for my sponsorship

- In form I-130 for the question "address where you last lived together" - what should we enter if we lived together for around 2 mos before she left for UK and the apartment lease did not have her name? are we required to provide a proof of this?

- Can anyone provide a sample affidavit of support that needs to be filed with form I-130

- Since I am already in US can we file I-130 and 485 together ? what is recommended and does filing it together expedite the case?
