Name change.

My wife recently became a US citizen. During her Citizenship interview, she requested to have her name changed from Nishaben Patel (name on Indian passport and Green Card) to Nisha Patel.

When filling out an I130, do I need to put her old name in other the field 'Other Names Used' or can I leave it blank.

If I fill in this field, do I need to show proof, and if so, where can I get this proof of name change from?

Yes, she needs to put her old name in other names used.

What exact process did she go through for the name change? Did she appear before the judge? They must given some paperwork regarding the name change. Submit a copy of that.
