My mom came here on B2 visit visa 4 months ago .Now she is going to apply for adjustment of status . This site is really very helpful site .I prepared all documents and paper work with the help of this site . I also got help with USCIS web site . But yesterday I visited another site and I am very upset to read there that a cover sheet with bar code is necessary for adjustment of status .As I say ,I consulted with USCIS web site and know that in cocurrent filling of I-130 and I-485 , all documents should send to Chicago lockbox .Pls you guys help me . What is this cover sheet and really is required in my mom case?
