ITIN number for Dependent Visa(H4)

Hi,

I reside in USA. I got married in February 2015. I would like to apply ITIN number for my wife( who is in Dependent H4 Visa ). I am unable to show her name in my income tax returns of last year( as I got married only this year ).
Could anyone please tell me the procedure and the documents that I need to share with IRS office to get an ITIN number for her ?
