PIO card for minor, need copy of all pages of passport?

Hi.

I'm a Indian citizen planning to apply for PIO card for my son who is minor born in US. On the PIO website it states that I need to submit the copy of my Indian passport. Do I need to make a copy of all pages of my passport or just the identity and address pages. Also it doesn't say that I need to get it notarized so I'm thinking it is not required.

Also it says that only Fedex and UPS should be used and not USPS even though USPS is reliable. I was wondering if anybody sent the application via USPS.

I will appreciate your help with the above questions.
