New OCI Card Application BLS International Checklist

BLS International webpage for OCI Card application under "Information" tab says that "Documents should be provided in the same order as they appear on this checklist". However this website does not provide a sample of checklist. I am going to apply to BLS in San Francisco. Does anyone know what this checklist is?

Thanks for your reply. So there is no checklist (form) like Travisa used to have when one printed part A & B online. When my wife applied for OCI couple of years ago she had to submit this checklist along with other required documents.

Useless Employees @ BLS

The BLS staff doesn't know what documents needed for OCI reapplication(To change the OCI Sticker to new passport). They are insisting to give a bundle of documents with all pages/copies of parents US & Old Indian Passports,renunciation certificate etc. though India Govt OCI reapplication/Miscellaneous service section is clear about the required documents,but BLS website requirements are different and if you go to BLS Office, their staff have their own requirements. It seems like these subcontractors of Indian external affairs is making life miserable for Indians.

Upon inquiry with BLS INTERNATIONAL San Francisco region they informed via email couple weeks ago that they are working on checklist and it will be uploaded soon. They asked me to submit application without checklist an I did mail documents on July 15, 2013. They returned all the documents back to me indicating that they need a police report for my lost Indian passport and without that the affidavit I sent was not acceptable. Well I did not see any requirement for police report on BLS website. Also I got my renunciation certificate without police report. Any suggestions if I can not get police report?
