Do I need a name change affidavit..or it is fine..??

Hi All,

In my Indian passport, I had my name as "xxx yyy zzz" as Given Name and " " in Surname. Now in my Canadian passport I have "xxx yyy" in Given Name and "zzz" as Surname. I am now getting ready for the Surrender Certificate, Renunciation and OCI.

Do I mention that I have changed my name..or its still the same..Do I need to go for a affidavit for name change..Confused.

I had a similar situation. I did an affidavit stating that both the names belong to the same person, and that I have not changed my name. I then got it notarized. Worked.

Yes you do need an affidavit for that, but it is very easy to get. Get it notarized from a Notary, In US you can get it at any bank. In Canada it might cost you $15-20 bucks to get it notarized. I have a template I am sharing here.affidavit-template.doc
