CK & G Oder Form Signature for Minors

Hi all,

Hope somebody has good experience with Ck & G. Let me come straight to my question. there are multiple form for PIO card in minor section. one of the form C k & g order form where we need to describe payment details. now at the end of this form there is place to apply signature for applicant now I got my daughter(3 months). so I was wondered who sign this form?

I did not find any description anywhere. everybody describe how to sign and thumbprint PIO form but no discussion for this form? please help me here in case anybody has gone through this situation. I have call them and ask and according to call center thumb print here also.

But I want to cross verify with everybody in this forum. I did not have really good experience last time, and I want to finish this time in one trip

I am also applying for my son. I am going with my signature since that form is more related to payment details.
