  • PIO Application submission at CKGS Chicago

    Hi - I am planning to submit the application througha walk in at Chicago Consultate and have some questions if you can help me with that.

    1. What are the dimensions of the photo needed. Confusion whether its a US Passport size ( 2 x 2) or a stamp size. If its stamp size then what are the dimensions that i am looking at. Also for an infant do you know if its alright to have the eyes closed in the photo (it works for US passport but not sure about the PIO)

    2. Do i need to submit only the PIO application form in 2 copies and rest are required as single copy only ?

    3. Do they accept credit card for payment?

    PIO Application submission completed at CKGS Chicago

    hi - Thought of coming back and sharing the experience.

    I submitted the PIO application for a minor at CKGS Chicago in person. The process took 40 mins and was smooth. i got there couple of hours before my appt. time but since the crowd was minimum, i was allowed to get the work done early. Here are answers to original queries i had


    1. 4 photos are required and they accept 2 x 2 size (US Passport size). I had the eyes of my kid open in the photo

    2. Only PIO application form is required to be in duplicate (with Page 3 of both copies notarized) and only 1 copy is required for the rest of the forms and supporting documents.

    3. They accept credit card also for payment.

      Do you know if parents should have valid visa stamped? We are in the process of applying PIO for our 4 month old.

      My visa - H1B - expired Sep 2013 - have valid I797 till 2016
      My wife's visa - H4 - expired Sep 2013 - have valid I797 till 2016

        Hi - i am not aware of this scenario. I would recommend that if you are unsure of any documents, it may be good to submit the application in person. Thats the only way to get the authentic information. The helpdesk telephone service is a waste.

          I think you should be good. if you have a valid I797 ..i had a friend whose condition was similar...he got PIO for his Son

            Thanks everyone for your help and suggestion. I called up customer service and they seemed to be confident that I797 should work as far its a "supporting document". Thought to share!
