OCI Reissue for Minor Questions

Can someone who has applied for OCI Reissue please help with the following questions?

1. I have already renounced Indian citizenship. Is a COPY of the Renunciation acknowledgement sufficient? No need to send original correct?
2. Although they ask for 2 copies of all documents, I presume that only one copy of the CKGS order form and Parental Authorization form is needed? Or do I need to send 2 copies of those too?
3. If I have ticked the checkbox for FEDEX return (15$) do I still need to include a prepaid envelope?
