1. RENUNCIATION CERTIFICATE
DECLARATION OF RENUNCIATION OF CITIZENSHIP OF INDIA ON
ACQUISITION OF CITIZENSHIP OF ANOTHER COUNTRY (Two Copies)
2. Deemed Surrender Certificate Declaration of Renunciation of Citizenship of India on Acquisition of Citizenship of another Country. (Two Copies)
3.Additional Particulars Form (for New York only)
4. CKGS Disclaimer Form

R u sure about declaration ?. I have to strike off (b) not as a spouse of Indian origin.

If i strike off b. that mean I am applying for a OCI card as a spouse of an Indian Origin., right?

I do not know what does a & b stand for?

I am sorry. Cross out as a spouse of Indian origin.
On US passport all the biographic info is on the first page so just include copy of that. and the cover at end of the passport and last two pages of the passport which in your case would be 28 and 27.

Is a photo required for renunciation? I am applying for OCI too but haven't seen the forms yet. I am sure a photo would be required for OCI. However, no forms for renunciation ask for a photo.

CGKS website clearly says Deemed Surrender certificate is required. Why do you say it is not required?
